Boston Primary Care Network (BPCN)
Swineshead Medical Group work very closely with the BPCN. We have staff allocated to the practice to support patient care.
The staff listed below work in within the practice providing additional support and care to our patients
Florence Adeboye - Clinical Pharmacist - works with James Musk (PCN Lead Clinical Pharmacist) to resolve day-to-day medicine issues and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medications and better access to health checks. The role is pivotal to improving the quality of care and ensuring patient safety.
James Smith - Community Psychiatric Nurse - James works across 3 GP practices in the Boston Primary Care Network. His role is to support patients with mental health issues.
Lisa Young - Care Coordinator - Lisa helps and supports our local care homes and provides admin supports to Caroline Bardwell our Older Adult Nurse.
Linda Brackenbury - Cancer Care Coordinator - Linda helps navigate patients who have a cancer diagnosis. She will also help with the cancer screening programmes such as smears, bowel screening etc.
Lisa Osborne - Trainee GP Assistant - Lisa will be helping with a variety of different aspects within the clinical team. She will assist the GP with urgent tests and administration.
